Service Dog Admission Criteria

Diversified invites participants to bring certified service dogs to our Mental Health Programs. Service
dogs must be certified by an accredited school to ensure they meet rigorous training standards. Our
Policy Statement guarantees reasonable accommodation for service dogs while ensuring the safety
and comfort of all participants.

Participants intending to bring their service dogs to Diversified’s Mental Health Programs must
present the following documents for verification:

A letter from a qualified healthcare professional (psychologist, or psychiatrist) stating the participant’s medical condition and the necessity of a service dog for assistance.

Proof of up-to-date vaccinations and licensing for the service dog as required by local authorities.

Documentation verifying the service dog’s training and certification from a recognized service dog school.
